About the Role
As a Dementia Nurse Specialist, you will provide clinical leadership and deliver person-centred, culturally responsive care across community and primary care settings. You’ll play a key role in care coordination, and ongoing support, helping people live well with dementia while reducing avoidable hospital admissions. This is more than a clinical role it's a chance to influence best practice, lead change, and make a real difference.
Key Responsibilties
- Conduct comprehensive dementia and cognitive assessments
- Develop personalised care plans that reflect individual and whānau needs
- Support early diagnosis and provide meaningful post-diagnostic guidance
- Educate and empower whānau, carers, and healthcare professionals
- Coordinate care across primary, secondary, and community services
- Lead behavioural and responsive care strategies
- Contribute to service development, innovation, and quality improvement
- Champion culturally safe, best-practice dementia care
- Maintain accurate, high-quality clinical documentation
About You
You are a skilled clinician who combines expertise with empathy and cultural awareness.
- Registered Nurse with a current APC
- Postgraduate qualification in mental health, or dementia care
- Proven experience supporting people living with dementia and their carers
- Strong understanding of dementia care pathways and assessment tools
- Excellent communication and stakeholder engagement skills
- Deep commitment to person- and whānau-centred care
- Able to work independently and collaboratively within multidisciplinary teams
