 About the Role

As a Dementia Nurse Specialist, you will provide clinical leadership and deliver person-centred, culturally responsive care across community and primary care settings. You’ll play a key role in care coordination, and ongoing support, helping people live well with dementia while reducing avoidable hospital admissions. This is more than a clinical role it's a chance to influence best practice, lead change, and make a real difference.

Key Responsibilties

  • Conduct comprehensive dementia and cognitive assessments
  • Develop personalised care plans that reflect individual and whānau needs
  • Support early diagnosis and provide meaningful post-diagnostic guidance
  • Educate and empower whānau, carers, and healthcare professionals
  • Coordinate care across primary, secondary, and community services
  • Lead behavioural and responsive care strategies
  • Contribute to service development, innovation, and quality improvement
  • Champion culturally safe, best-practice dementia care
  • Maintain accurate, high-quality clinical documentation

 About You

You are a skilled clinician who combines expertise with empathy and cultural awareness.

  • Registered Nurse with a current APC
  • Postgraduate qualification in mental health, or dementia care
  • Proven experience supporting people living with dementia and their carers
  • Strong understanding of dementia care pathways and assessment tools
  • Excellent communication and stakeholder engagement skills
  • Deep commitment to person- and whānau-centred care
  • Able to work independently and collaboratively within multidisciplinary teams

Applicants must be a New Zealand Citizen or Permanent Resident.
